Как будет работать новая система медицинского обслуживания

Всего четыре месяца осталось до начала масштабной реформы кипрского здравоохранения – обязательного введения Генеральной системы медицинского обслуживания (ГЕСИ). А уже через два месяца государство начнет вычитать из доходов населения взносы на реализацию этого нововведения.

Основной принцип реформы – сделать медицинскую помощь доступной для каждого. Каким образом? Не секрет, что высокие расценки на услуги частных врачей для многих являются непосильными. Чтобы сгладить это социальное расслоение, государство вводит взносы, которые будут вычитаться из доходов населения. Эти взносы будут идти на зарплаты врачам. Благодаря этому пациентам уже не придется платить докторам за каждый визит. Таким образом, медицина станет доступной для всех.

Второй основной принцип – введение института «личного врача». Каждый гражданин, который имеет право вступить в ГЕСИ (граждане Кипра, граждане ЕС, граждане третьих стран, имеющие право на долгосрочное проживание) должны по своему выбору «прикрепиться» к врачу общей практики. Этот врач становится их личным доктором. Сам врач в свою очередь ведет реестр прикрепленных к нему пациентов.

Средняя сумма жалованья для личного врача общей практики составит 135 тысяч евро в год, для специалистов – 160 тысяч.

Споры о ГЕСИ ведутся по многим пунктам, начиная с того, сколько бюджетных средств потребуется на реализацию реформы. По данным Организации медицинского страхования (ОМС – именно эта организация ответственна за воплощение реформы в жизнь), нововведение обойдется в 1 млрд евро. Однако Кипрский союз врачей уверяет, что сумма занижена – по его подсчетам реформа будет стоить 1,6 млрд.

Вознаграждения и предложения

Кипрский союз врачей обвиняет ОМС в том, что все предложения врачебного сообщества по улучшению хода реформы были отклонены. ОМС назвала эти обвинения «не соответствующими реальности» и заявила, что план реформы и так сам по себе отвечает всем требованиям, кроме одного – права для врачей, вступивших в ГЕСИ, заниматься частной практикой, так как частная практика идет вразрез с основными принципами реформы. «ОМС дала свой ответ на все существенные вопросы, среди которых увеличение бюджета и противодействие слишком большому количеству пациентов», – говорит представитель организации.

Предложения ОМС для личных врачей

Для педиатров дела обстоят следующим образом: за каждого ребенка, внесенного в реестр, врач будет получать следующие суммы (в год) – 210 за евро за ребенка до 3 лет, 155 евро за ребенка 4-7 лет, 91 евро за подростка 8-18 лет.

Сопровождение родов и осмотр новорожденного будут вознаграждаться суммой в размере 150 евро. Если врач должен будет работать сверхурочно (с 20:00 до 8:00, а также в выходные и праздники), он будет иметь право взять с самого пациента 25 евро дополнительно.

Если педиатр, ведущий частную практику, примет решение вступить в ГЕСИ и «приведет друга», то есть будет вести прием в одном офисе с другим педиатром, то ему единовременно будет выплачено 15 тысяч евро. Кроме того, перед началом работы системы ОМС совместно с Кипрским союзом врачей определят верхнюю границу посещений для разных групп пациентов (исходя из возраста и наличия хронических заболеваний), чтобы избежать перегрузки врачей на рабочем месте.

Зарплаты врачей общей практики

За каждого пациента в возрасте от 16 до 50 лет врач будет получать в год 83 евро, за пациента в возрасте 51-70 лет – 117 евро, за пациентов старше 70 лет – 145 евро. Стоимость обследования тех пациентов, которые не внесены в реестр, врач сможет назначать самостоятельно. При посещении пациентов на дому стоимость услуг также будет назначаться самим врачом.

Вознаграждение за граждан других стран

За прием граждан других стран Европейского союза врач будет получать от ОМС 35 евро. А сумму за обследование граждан третьих стран он будет назначать сам.

Стационарное лечение

Врачи смогут курировать своих пациентов во время нахождения тех в стационаре. Это также будет оплачиваться отдельно. Размер вознаграждения сейчас обсуждается Организацией медицинского страхования и Кипрской ассоциацией частных клиник.

Права пациентов и врачей

Родители смогут беспрепятственно сменить педиатра в течение первых трех месяцев жизни своего ребенка. В возрасте от трех до 24 месяцев педиатра можно будет менять каждые три месяца. После 2 лет поменять врача можно будет каждые 6 месяцев. Также и врач будет иметь право отказать в обслуживании пациента (кроме случаев, когда такой отказ может быть воспринят как дискриминация).

Финансирование ГЕСИ

Взносы на функционирование новой системы здравоохранения будут производиться из доходов сотрудников, работающих по найму, самозанятых, работодателей, пенсионеров, предпринимателей, чиновников и государства. Бюджет реформы оценивается почти в 1 млрд евро. Сегодня на Кипре примерно 7% ВВП тратится на нужды здравоохранения. Это ниже среднего уровня по ЕС.

Проблемы при финансировании

Компания Deloitte, которая провела свой анализ затрат на реформу, пришла к выводу, что бюджет на ее реализацию может быть занижен, так как в своих расчетах государство основывалось на докризисных показателях 2013 года. По их мнению, нужно исходить из того, что реформа обойдется примерно в 9-10% от ВВП. Именно таков средний показатель расходов на здравоохранение в государствах ЕС.

Условия врачей для вступления в ГЕСИ

На экстренном общем собрании Кипрского союза врачей был принят список из пяти условий, которые должны быть выполнены, чтобы врачебное сообщество приняло новую систему:
1. увеличение бюджета реформы до средних по Европе показателей (9-10% от ВВП);
2. принятие закона, в котором было бы написано, что «административная и экономическая автономизация государственных клиник является предварительным условием для внедрения ГЕСИ»;
3. принятие графика реализации реформы с учетом интересов государственных и частных врачей, государственных и частных клиник, ради обеспечения безопасности, качества и эффективности системы;
4. установление размера жалованья личных врачей согласно рекомендациям Кипрского союза врачей;
5. установление точных расценок на все действия врача во время посещения пациента в стационаре.

Пока кипрские врачи ставят свои условия государству и отказываются идти на компромисс, новой системой заинтересовались врачи из Греции. Это и не удивительно, так как уровень зарплат, от которых так легко отказываются киприоты (примерно 10 тысяч евро в месяц), для греков является недосягаемым. Для сравнения, жалованья врачей без опыта работы колеблются от 1 161 до 1 903 евро, а максимальный показатель зарплаты достигает 2 908 евро – столько получает главврач с опытом работы более 29 лет.

    1            Application of terms

    1.1        These Terms apply to your use of the Website. By accessing and using the Website:

    a            you agree to these Terms; and

    b            where your access and use is on behalf of another person (e.g. a company), you confirm that you are authorised to, and do in fact, agree to these Terms

    on that person’s behalf and that, by agreeing to these Terms on that person’s behalf, that person is bound by these Terms.

    2.1        We may change these Terms at any time by updating them on the Website. Unless stated otherwise, any change takes effect immediately.     3            Definitions

    In these Terms:

    including and similar words do not imply any limit

    Loss includes loss of profits, savings, revenue or data, and any other claim, damage, loss, liability and cost, including legal costs on a solicitor and own client basis

    personal information means information about an identifiable, living person

    Terms means these terms and conditions titled Terms and conditions of use

    Underlying System means any network, system, software, data or material that underlies or is connected to the Website

    We, us or our means VESTNIK KIPRA Group

    Website means https://vkcyprus.com/

    You means you or, if clause 1.1b applies, both you and the other person on whose behalf you are acting.

     

    4            Your obligations

    4.1        You must provide true, current and complete information in your dealings with us and must promptly update that information as required so that the

    information remains true, current and complete.

    4.2        You must:

    a not act in a way, or use or introduce anything (including any virus, worm, Trojan horse, timebomb, keystroke logger, spyware or other similar feature) that in any way compromises, or may compromise, the Website or any Underlying System, or otherwise attempt to damage or interfere with the Website or any Underlying System; and

    b unless with our agreement, access the Website via standard web browsers only and not by any other method. Other methods include scraping, deep-linking, harvesting, data mining, use of a robot or spider, automation, or any similar data gathering, extraction or monitoring method.

    4.3 You indemnify us against all Loss we suffer or incur as a direct or indirect result of your failure to comply with these Terms.
     

    5            Intellectual property

    We (and our licensors) own all proprietary and intellectual property rights in the Website (including all information, data, text, graphics, artwork, photographs, logos, icons, sound recordings, videos and look and feel), and the Underlying Systems.

    6.1        To the extent permitted by law, we and our licensors have no liability or responsibility to you or any other person for any Loss in connection with:

    a the Website being unavailable (in whole or in part) or performing slowly;

    b any error in, or omission from, any information made available through the Website;

    8            Privacy policy

    8.1        You are not required to provide personal information to us, although in some cases if you choose not to do so then we will be unable to make certain sections of the Website available to you. For example, we may need to have your contact information in order to provide you with updates from our Website.

    8.2        The personal information you provide to us is collected and may be used for communicating with you, statistical analysis, the marketing by us of products

    and services to you, credit checks (if necessary), and research and development.

    10         General

    10.1      If we need to contact you, we may do so by email or by posting a notice on the Website. You agree that this satisfies all legal requirements in relation to written communications.

    10.2      These Terms, and any dispute relating to these Terms or the Website, are governed by and must be interpreted in accordance with the laws of the

    Republic of Cyprus.

    Each party submits to the non-exclusive jurisdiction of the Courts of the Republic of Cyprus in relation to any dispute connected with these Terms or the

    Website.

    10.3      For us to waive a right under these Terms, the waiver must be in writing.

    10.4      Clauses which, by their nature, are intended to survive termination of these Terms, including clauses 5, 6, 7, 10.1, continue in force.

    10.5      If any part or provision of these Terms is or becomes illegal, unenforceable, or invalid, that part or provision is deemed to be modified to the extent

    required to remedy the illegality, unenforceability or invalidity. If a modification is not possible, the part or provision must be treated for all purposes as

    severed from these Terms. The remainder of these Terms will be binding on you.

    10.6      These Terms set out everything agreed by the parties relating to your use of the Website and supersede and cancel anything discussed, exchanged or

    agreed prior to you agreeing to these Terms. The parties have not relied on any representation, warranty or agreement relating to the Website that is not

    expressly set out in the Terms, and no such representation, warranty or agreement has any effect from the date you agreed to these Terms.
